New Patient Information
Your First Visit
Thank you for considering Westgate Dental Care to provide dentistry services for you and your family. Before your visit, remember our team is here to help you get ready, so don’t hesitate to contact us with questions. At your first visit, we’ll take time to make sure you feel at home by giving you a tour of the office, and reviewing your new patient information forms. During the treatment phase of your visit, we’ll carefully examine your teeth and gums, screen for warning signs of common concerns, and provide a thorough teeth cleaning. Then, you’ll sit down with one of our skilled dentists to review our findings and create a plan for your ongoing oral health care. It’s always our goal to make patients of any age or stage of dental development feel right at home in our office, so please don’t hesitate to let us know if we can help you feel more comfortable or relaxed.
In order to expedite your first appointment, we ask that you allow an extra fifteen minutes prior to your scheduled appointment time to complete new patient forms, or you can simply download and fill out this documentation prior to your visit from the comfort of home. Many of our patients find that it’s much simpler to fill in this information at home where they have easy access to necessary documents. If you have questions while completing your new patient forms, please don’t hesitate to contact our office.
Our knowledgeable team members have years of experience processing and filing insurance claims to maximize patient benefit, and we’ll be happy to help you get the benefits you deserve from your PPO insurance plan. We are also in-network dental care providers for many of the most popular plans. If you have questions about your specific coverage, please don’t hesitate to ask. We’ll review policy documents and provide accurate estimates for coverage to the best of our abilities.
We ask that patients be prepared to remit payment at the time of service for the out of pocket costs of their treatment, and we accept payment in the form of cash, check, or major credit card. If you’d like to spread the out of pocket costs of your dental care over the course of several payments, we work with CareCredit and Chase Health Advance to provide low and no interest financing plans for qualified patients. Both companies specialize in financing for medical and dental care providers. We also accept payments online.
